What Happens After You Schedule?
Once your appointment has been scheduled, we’ll send you a text message and email with a secure link to your Patient Registration forms.
You can complete your forms on your phone, tablet, or computer, whatever is most convenient for you.
Complete Your Forms Before Your Appointment
Please complete your Patient Registration forms before your appointment whenever possible.
Completing your forms in advance helps us:
- Prepare for your visit before you arrive
- Reduce check-in and waiting room time
- Make your visit more efficient
- Spend more time focused on your care
Once your registration forms are complete, you’ll be directed to information to help you prepare for your upcoming visit.
